Can echocardiographic parameters effectively predict elevated pcwp in heart transplant patients?

Abstract Funding Acknowledgements Type of funding sources: None. PCWP measurement remains the gold standard for diagnosis Heart Failure with preserved ejection fraction. Nevertheless, right heart catheterization (RHC) is a last step procedure nowadays, most patients diagnosed solely by transthoracic echocardiography (TTE) parameters, as it widely available and not associated risks invasive procedures. transplant (HT) patients, on other hand, routinely perform RHC follow-up rejection surveillance, allowing frequent hemodynamic assessment. Purpose Evaluate association between TTE parameters invasively measured pulmonary capillary wedge pressure (PCWP) in HT patients. Methods A retrospective analysis consecutive submitted to February 2016 November 2021, who previously performed TTE, was made. The area under curve (AUC) evaluate several (left ventricular (LV) end-diastolic diameter (LV EDD), LV end-systolic ESD), fraction EF), interventricular septum wall thickness (ST), global longitudinal strain (GLS), left atrium volume index (LAVi), transmitral E/A ratio (E/A), E/e’ (E/e’), E velocity deceleration time (DT), isovolumetric relaxation (IV RT), tricuspid annular plane systolic excursion (TAPSE) artery (sPAP)) elevated (defined >15mmHg). Statistical differences p-value <0.05 were considered significant. Results total 127 during study period. Mean age 50 years, 79% (n=100) male, mean EF 58 ± 10% BNP 808 984 pg/mL. 12 6 mmHg, 28% (n=35) value higher than 15mmHg. AUC results prediction are depicted Table 1. (AUC 0.702, p = 0.001) GLS 0.768, 0.07) significantly correlated PCWP, while remaining failed do so. Conclusion In this group, indices commonly employed non-invasive estimation increased intraventricular pressures, including E/e', ratio, LAVi, sPAP, showed poor correlation PCWP.
